What can I claim when I refer for a Home Medication Review?

When referring a patient for a HMR, an MBS Item 900 can be claimed on top of your standard health assessment Item 703 on a regular visit.

 

The following is a suggested Annual Care Cycle for Chronic Disease Patients:

 

Month 1
Visit 1: Standard health assessment- 703 (referral for HMR
as part of this and recommendation that a GPCP be set up
for patients with chronic diseases) Fee: $130.10 Benefit:
100% = $130.10

Month 3
Visit 2: Preparation of a GP Management Plan- (MBS Item
721 Fee: $136.05 Benefit: 75% = $102.05 100% = $136.),
review and claim HMR (Item 900- Fee: $146.00 Benefit:
100% = $146.00)
